GRIDIRON NATION — PLAYER ASSUMPTION OF RISK, RELEASE & WAIVER

Important: This document concerns legal rights and potential claims. Read it carefully before signing. Do not sign if you do not understand it; ask questions and seek independent legal advice if needed.

1. Activity and acknowledgment of risks

I voluntarily choose to participate in Gridiron Nation Flag Football League activities (“Activities”), including evaluations, practices, tryouts, games, tournaments, warm-ups, conditioning, and related league events. I understand flag football is a physical, competitive activity involving inherent and other risks, whether known or unknown.

Risks include, but are not limited to, collisions and contact with other players; falls, trips, slips and impacts with the ground or equipment; being struck by a ball or another person; sprinting, cutting, jumping, twisting and sudden stops; overexertion, dehydration, heat illness, weather exposure and exhaustion; and injuries such as sprains, strains, fractures, dislocations, concussions, head, neck or spinal injuries, heart-related events, permanent disability or death. I understand that flag rules and the use of flags do not eliminate physical contact or these risks.

Risks may arise from my own actions, actions or inaction of other participants, playing surface or facilities, weather, equipment, officiating, or event organization. I voluntarily accept the ordinary and inherent risks of participating.

2. Player responsibilities and fitness

I agree to follow league rules, safety instructions, officials’ directions, facility rules and the code of conduct; use required and properly fitted equipment; avoid prohibited or reckless contact; treat players, officials, staff and spectators respectfully; and promptly report unsafe conditions, injuries or symptoms. I will not participate while impaired by alcohol, illegal drugs or any substance that makes participation unsafe.

I represent that I am at least 18 years old and believe I am physically able to participate. I am responsible for deciding whether to participate and for obtaining medical advice about any condition that could affect my safety. I will stop participating and notify league personnel if I experience pain, dizziness, shortness of breath, confusion or other concerning symptoms.

3. Release of liability — limited by applicable law

To the fullest extent permitted by applicable law, I release and agree not to bring a claim against the league operator and its owners, organizers, directors, officers, employees, volunteers, coaches, team representatives, officials, event staff, sponsors and participating facility owners/operators (collectively, the “Released Parties”) for injury, illness, death or property damage arising out of or related to my participation in the Activities, including claims based on the ordinary negligence of a Released Party, if and to the extent such a release is enforceable under applicable law.

This release does not apply to, and I do not release, claims that cannot lawfully be waived, including claims based on a Released Party’s wanton or willful misconduct, or any other conduct that applicable law does not permit me to release. The scope and enforceability of a pre-injury release depends on applicable law and the facts. Nothing in this document is intended to waive a right that cannot legally be waived.

4. Medical emergency authorization

If I become injured or ill and cannot make or communicate a decision, I authorize league personnel to contact emergency services and request emergency evaluation or transport. I understand league personnel are not medical professionals and are not required to provide medical treatment. I am responsible for medical, ambulance, hospital and related expenses not covered by my insurance. This authorization does not require anyone to provide treatment or delay emergency care.

5. Personal property and insurance

I am responsible for my personal belongings and understand the league does not provide personal health, accident or property insurance for me unless expressly stated in writing. I am encouraged to maintain my own health and accident coverage.

6. Photo and video permission — optional

My choice below about use of photographs or video for league-related promotion, website, social media and recap materials is separate from my agreement to participate.

7. General terms

I have had the opportunity to read this document, ask questions and decide whether to sign it. I understand it is intended to be legally binding to the extent permitted by law. If any provision is found unenforceable, the remaining provisions will remain in effect to the extent allowed by law. This document is governed by the laws of the State of Alabama, without regard to conflict-of-law rules, subject to any mandatory law that applies. This document does not replace the league rulebook, code of conduct, registration form or any separate facility agreement.
